Health History Questionnaire

Nova Scotia Health provides a preoperative health history questionnaire to patients before most types of surgery. It asks about things like: medications or supplements, allergies and other health problems, mobility issues, past surgeries, and supports you may need after surgery.

Step 1 - Patients are asked to fill out the questionnaire before they see the surgeon in their office

They can do this on their own, with a loved-one, friend, or with help from a care provider.

It is important patients answer the questions as best they can.

The information they share helps their health care team plan for their safety and care.

Step 2 - Our health care team reviews the forms to decide if patients will need to have tests or visit a clinic before their surgery.

Tests such as bloodwork may be ordered.

Some patients may also be booked for a pre-operative or  perioperative anesthesia clinic. These clinics help us collect and share more information to help plan for a patient’s surgery.

Step 3 - Patients complete any tests or clinics that are needed and get information to prepare for the surgery.

Health care providers go over the patient’s file and work with them to finalize plans for their surgery.

Patients may be given special instructions. For example, they may be told to stop using a medication before surgery.
