Bayers Lake Community Outpatient Centre

This clinic forms a group of local primary care providers (family doctor or nurse practitioner) who rotate to provide care to each other’s patients after hours, on evenings or weekends. These clinics are only available to patients who belong to the primary care providers who are participating. Patients must book appointments by calling the clinic. No walk-in patients are accepted.

How do I change or cancel an appointment?
Please call toll free at 1-844-I-REBOOK (1-844-473-2665). You will be asked to select the location of your appointment and service.

You will be connected with a team member who can cancel your appointment and help arrange for a new appointment.

If you are a patient with moderate or advanced COPD who meets our eligibility criteria and would like to be part of the The INSPIRED COPD Outreach Program, please ask your family physician or respirologist to fill out the INSPIRED COPD outreach Program referral form.

Appointments are made upon referral only.

Referrals are accepted by family doctors, other specialty doctors, other ophthalmologists and optometrists. Urgent referrals are accepted as well, usually triaged by the Red Eye clinic team.

You will need a referral from the Orthopedic Assessment Clinic.

A completed referral form and updated X-rays are required to process the referral. Primary health care providers will need to arrange to have X-rays completed within one year, with specific views as indicated on the referral form.

Upon entering the main doors to the Bayers Lake Community Outpatient clinic you will register with central registration for your MRI . You can use the qmatic kiosks to generate a number so you can be queued to see a registration clerk. Upon registering you will then be directed to the DI waiting area where you fill out an MRI safety screening form and get changed in preparation for your MRI. 

Occupational Therapists are primary care professionals (just like doctors and dentists) which means you can complete an Outpatient Self Referral to go directly to an occupational therapist. In some circumstances your physician may be required to complete a Physician Outpatient Referral Form -  on your behalf.

If your primary care provider (family doctor, nurse practitioner etc.) believes you may benefit from joint replacement surgery they will make a referral to an Orthopedic Assessment Clinic. After receiving their referral the OAC team will schedule an appointment with you

In some circumstances your physician may be required to complete an Outpatient Physiotherapy Referral on your behalf.
