There may come a time when you or someone you know needs help to live well at home or in the community for as long as possible. To access home and community care services – such as nursing care or help with bathing, preparing meals and other daily activities – call 1-800-225-7225 (toll-free).

A member of our Care Coordination team will help to arrange care services.

We also encourage you to view A Guide to Receiving Home Care Services in Nova Scotia.

Home First Approach

At Nova Scotia Health, we take a Home First approach. Evidence shows that healing happens better at the place you call home. Home First is more than a philosophy, it's about:

  • creating a discharge plan with the goal of getting you back home.
  • providing the care you need to keep living at home.

We start planning for your discharge home as soon as you are admitted to the hospital. Our goal is to connect you with the right community resources to support your recovery, independence and quality of life outside of the hospital.

Continuing care programs and services are one of the many supports that provide care at the place you call home.

Sharing What Matters with your Care Providers

When your care team knows as much about you as possible, they can provide the best care. One way you can help them get to know you is by filling out a Social History and sharing it with them. Please consider filling out this form and sharing it with your care team (or you can ask us to give it to them for you). Learn more about this voluntary form.

New Health Information System

Nova Scotia Health's new health information system will continue to roll out beginning November 14th, 2026, at all facilities in the Northern, Eastern and Western Zones.

During this time of transition to a better patient-record system, our teams will keep working to get you the care you need.

You may notice staff in coloured vests and t-shirts helping our care teams adapt to the new system.

We appreciate your patience and understanding.
