Care Coordination Centre (C3) technology expands in rural Cape Breton
At Sacred Heart Community Health Centre in Cheticamp, staff take pride in delivering care to their close-knit community. Now, thanks to new technology, they’re even better equipped to support patients and each other.
Sacred Heart recently adopted Nova Scotia Health’s Care Coordination Centre (C3) technology and is now using the Patient Manager tile.
This tool gives healthcare teams a clear, shared view of each patient’s needs, such as what procedures must be completed before patients return to the place they call home, when patients are scheduled for transfer to appointments, and anticipated discharge dates. Having reliable, up-to-date information available digitally in one place helps staff make informed decisions and stay aligned throughout their shifts.
Johanne Doucet is site lead at Sacred Heart Community Health Centre. “We hope patients feel reassured knowing we have tools to closely monitor their care, communicate effectively, and respond quickly. The Care Coordination Centre Patient Manager tile has been a valuable addition to our site,” she said.
In rural sites like Sacred Heart, where team members often wear many hats, the C3 system is already making a meaningful difference, helping staff streamline communication, anticipate potential issues earlier, and co-ordinate care more efficiently.
The technology supports what makes the community special. “We’re a small, rural site where staff know their patients and community well,” Doucet said. “The C3 technology supports that uniqueness by giving us a tool that enhances collaboration between our team members, and it helps us maintain high standards of care.”
To prepare for the change, the Care Coordination Centre team worked closely with staff, offering hands-on guidance and education. This helped quickly build confidence and ensured the technology was implemented in a way that meets the needs of a rural care setting. “We’re proud to be using this tool and we look forward to continuing to build on its benefits as we work to ensure our patients receive the care they need right here in their community,” Doucet said.
