Spotlight on Charlotte Green, Practice Facilitator in Northern Zone

Meet Charlotte Green, a Primary Health Care Coordinator working as a Practice Facilitator in the Northern Zone at Nova Scotia Health. With a background in Psychology and Addictions Counselling, Charlotte has spent six years with Nova Scotia Health, including five years working with the Early Intensive Behavioral Intervention Program. Now, as a Practice Facilitator, her role focuses on making primary healthcare delivery more efficient for both staff and patients. 

 

Charlotte’s work can vary from day to day, but one of her favorite tasks is helping clinic staff optimize their use of the electronic medical record (EMR) system. "When staff feel confident with the technology, everything runs smoother," she explains. 

 

Beyond her work with digital health technology, Charlotte thrives on building strong relationships with primary healthcare providers and teams and contributing to staff education by introducing new tools and facilitating training sessions. For her, the impact goes beyond the data she collects. "The most rewarding part of my job is making someone’s day a little easier,” she says, showing her passion for simplifying healthcare’s complexities and making things easier for those she supports. 

 

Practice Facilitators like Charlotte play a crucial role in primary care by working directly with clinics to improve operational workflows, support clinic teams, and ultimately enhance patient outcomes. By streamlining these processes, Practice Facilitators help clinics function more efficiently, making healthcare more accessible to the communities they serve. 

 

Charlotte's commitment to empowering others extends beyond her professional role. In her free time, she coaches youth hockey and softball teams, applying the same principles of teamwork and steady progress that she uses in the clinic. Whether on the field or in the office, Charlotte understands that with collaboration and small, consistent steps, big wins are always possible.
