Have you ever wondered how healthcare is shaped behind the scenes or how technology is driving change in the way we care for Nova Scotians like you?

Fellows from the Medventions Atlantic program visited the Care Coordination Centre (C3) provincial hub to learn more about how new and advanced technology is changing the way we care for patients across the province.

The Medventions Atlantic fellowship is a partnership between the Nova Scotia Health Innovation Hub and Sunnybrook Research Institute. The goal is to nurture Canadian talent in the healthcare technology sector. The fellowship connects students and new graduates with innovators, clinicians, researchers, experienced mentors and innovative initiatives like C3.

The provincial C3 is the central main hub that manages patient movement and flow within and between hospitals throughout the province. Coordinating with local hubs across the province, it guides patients through their healthcare journey faster by using advanced technology. C3 enables teams to follow patient progress through hospitals, and aims to reduce barriers by filling open appointments, coordinating transfers, as well as tracking and filling open beds, and more.

Jacqueline DeJarnette and Christian Moya-Garcia, two Medventions winter 2025 fellows, were guided through how the technology works by C3 provincial training manager, Stacey Pyke. She shared how patient information is tracked using C3 technology and how healthcare professionals use it to make quicker and better care decisions.

“We were delighted to welcome the recent group of talented and innovative fellows from Medventions Atlantic to our provincial C3 location. Being a part of their learning journey and engaging with such bright minds who are dedicated to advancing healthcare innovation was a wonderful experience for our team. Their insightful questions reinforced our shared commitment to enhancing patient care” says Pyke.

Before the visit, Jacqueline shared that she hadn’t realized how detailed the data from C3 could be. “C3 technology really shows how complex patient flow, coordination, and care can be,” Jacqueline says “It was insightful to see technology and people collaborate to make the system work smoothly.”

The fellows were impressed by how the technology follows each patient who enters the healthcare system. Drawing on their firsthand experience, they were able to connect the technology to the practices they have seen and recognized its potential to transform, expand, and improve healthcare.

Christian expressed how valuable it was to see the information compiled by C3. He recognized the importance of moving patients strategically to ensure that those who need care the most are prioritized. “I was really inspired by the real-time data at their fingertips,” Christian said. “It allows for quicker responses when patients are flagged in the system. I also appreciate how the team continues to collaborate with the system provider to improve the technology's functionality.”

C3 ensures we have the right people in the right place at the right time. It provides a real-time view of access to care, patient flow, and health system resources to support decision-making and provide patients with the care they need when they need it.
