A physician or nurse practitioner's referral is required (sent by fax to 902-543-8895) and you must be a patient with a history of:
- angina pectoris
- acute coronary syndrome (heart attacks or unstable angina)
- percutaneous coronary intervention (stenting)
- coronary bypass surgery, valve replacement or cardiac transplant
- peripheral arterial disease
- cerebrovascular accident (brain attack) or transient ischemic attacks (mini stroke)
- chronic heart failure and/or cardiomyopathy
- Or at least three major cardiovascular risk factors (based on the 2009 Canadian Cardiovascular Society Guidelines) (smoking, diabetes, hypertension, dyslipidemia, family history of coronary artery disease in relatives under age 60).
