Please plan for extra travel time when coming to the QEII Halifax Infirmary. As of February 18, the easternmost, northbound lane of Robie Street will be closed for the duration of the Halifax Infirmary Expansion Project, leaving two lanes open. Expect potential delays and consider alternative routes when possible.

Application Process for Agreement for Acceptance of Specimens Collected by Independent Phlebotomists for Laboratory Testing by Pathology and Laboratory Medicine

The steps below outline the process for establishing an Agreement for Acceptance of Specimens Collected by Independent Phlebotomists for Laboratory Testing by Pathology and Laboratory Medicine.

Step 1: Review the Agreement for Acceptance of Specimens Collected by Independent Phlebotomists for Laboratory Testing by Pathology and Laboratory Medicine to ensure an understanding of the requirements within the Agreement.

Step 2. Open the Independent Phlebotomy Application Form and save a copy.

Step 3. Complete the form. Please ensure all requested information is provided to avoid delays in processing. Forward the completed form as an email attachment to IPOffice@nshealth.ca . Keep a copy for your records. 

Step 4. The requestor will be notified if the application is approved, not approved, or incomplete.

Step 5. The application will be processed and an Agreement will be drafted and emailed to the requestor. Print and review the Agreement to ensure the information is complete and correct. Ensure all signatures are provided, compliance with the terms of the agreement, and all required certifications and supporting documents (proof of access to required CLSI guidelines, competency assessment, etc.) are prepared.

Step 6. Signed contracts and supporting documentation must be submitted to Pathology and Laboratory Medicine. Scan and email all documents to IPOffice@nshealth.ca .

Step 7. NSH and IWK authorized signatures will be obtained, giving final approval. The Agreement will be activated and laboratory site management notified with the activation date. A copy of the signed Agreement will be forwarded to the requestor by email, along with notification when collection can begin.

The process to review, approve and activate a new Agreement can take several weeks. Please ensure all requested information is provided to avoid additional delay.
