Les soins continues encourage et soutien votre bien être et votre indépendance. Lorsque vous faites appel à nous, nous évaluons vos besoins et nous cherchons avec vous les solutions de soutien adéquates, puis nous autorisons et organisons vos services. Les coordinateurs et coordinatrices de soins continus constituent le principal point de contact des clients et des familles pour obtenir des conseils et accéder aux services.

Note: In order to allow clients to benefit from our recommendations, a primary care provider who can implement recommendations, support and monitor the patients’ health is required prior to being assessed by the Seniors’ Clinic.
