Lorsque vous faites appel à nous, nous évaluons vos besoins et nous cherchons avec vous les solutions de soutien adéquates, puis nous autorisons et organisons vos services. Les coordinateurs et coordinatrices de soins continus constituent le principal point de contact des clients et des familles pour obtenir des conseils et accéder aux services.
