This program is designed for drivers of all ages who have experience a health change that could affect their ability to drive safely. It involves an assessment of the client’s physical, cognitive and perceptual abilities and includes an on-road evaluation.
To be seen by our service a written referral from a doctor is required. Please use the Driver Evaluation Referral Form. The referral can be faxed or mailed directly to our department.
