The Northern Zone Mobile Health and Wellness Team travels throughout Colchester, East Hants and Cumberland to provide health and wellness-based services in rural community areas. They also see some patients from their office in Truro if needed.
They can support minor physical ailments and provide healthcare navigation information and resources. They can also provide one-on-one and group programs to best meet your goals.

How do I access this service
Anyone can access this service via drop-in, self-referral for wellness groups and individual counselling or you may be referred by your provider.

Please bring your health card and contact information with you so that our team can reach out for follow-up as necessary.
What will happen at my appointment?
You will be greeted by our multidisciplinary team. We will determine your needs and complete an intake with you to gather relevant information. We will provide you with relevant resources and information and offer you the option to be seen by one or more providers. They will aim to address your concerns on the spot, but may offer to follow up in-person at our office or with a virtual follow-up.
In person follow-up may involve booking an individual appointment to see a provider in our office space or a phone call to review any outstanding concerns. You may be contacted directly by the provider or by our administrative support.
Who are the members of the care team?
This team consists of the following providers who can provide the following healthcare services:
Dietitian
Dietitians are registered healthcare professionals who use their expertise in food and nutrition to enhance lives and improve health.
They can:
- Support understanding the connection between food and health across your lifetime
- Conduct nutrition assessment
- Develop personalized intervention to meet your needs and goals
- Help prevent and manage chronic diseases such as diabetes, heart disease, and obesity
- Offer guidance on dietary changes during different life stages, such as pregnancy, breastfeeding, and aging
Emotional Wellness Counsellor/Navigator
Emotional Wellness Counsellors and Navigators are Social Workers or Registered Counsellors with an understanding of community resources and a background with coaching/counselling.
An Emotional Wellness Counsellor/Navigator can:
- Help you get connected to community and health resources for needs you identify.
- Facilitate wellness programs that support your mental wellbeing.
- Help you learn skills and strategies for regulating thoughts, emotions and behaviors, with the goal of improving your emotional health and overall wellbeing.
Family Practice Nurse
Family practice nurses are registered nurses with special training in primary health care.
They can:
- Help manage health issues such as diabetes, asthma, COPD, high blood pressure, smoking, cholesterol, and heart disease.
- Provide well-baby visits and prenatal care.
- Perform pap tests and yearly exams.
- Provide teaching and support for healthy living. Topics could include healthy eating, physical activity, mental health, and sexual health.
- Review and promote health screenings.
- Provide therapeutic interventions such as wound care, medication reviews and more.
Occupational Therapists
Occupational Therapists are registered healthcare professionals that use their expertise in assessment and interventions for everyday activities to support individuals and their communities.
OTs can conduct assessment of daily skills and activities in many areas, including:
- Mobility, such as balance, stairs and walking
- Self-care, such as eating, dressing and bathing
- Productivity, such as cooking, laundry, volunteering, finances and transportation
- Leisure, such as hobbies, outings, and social visiting
- Develop personalized treatment to support your priorities and goals
- Offer guidance for building skills in many areas, such as falls prevention, staying physically and cognitively active, and seasonal safety
Pharmacist
- Minor Ailments Assessing and Prescribing (cold sores, acid reflux, thrush, pink eye, hemorrhoids, impetigo, warts, acne and more)
- Chronic Disease Management visits- Diabetes, COPD, Cardiovascular disease
- Immunizations and injections -assessment and administration
- Smoking cessation programs
- Prescribing for UTI, Birth Control
- Prescription renewals* (except for narcotic/controlled)
- Ordering lab work to monitor therapy**
- Medication reviews (thorough review of your medications, their intended purpose etc)
- Prescribing for lyme disease prevention
- Assessing and prescribing for shingles
*Patient must have an active prescription for a condition that was previously diagnosed by a physician or NP
**Labs can only be ordered to monitor medications the patient is currently taking , not to diagnose new medical conditions
Social Worker
Social Worker’s in Primary Health Care are registered professionals that can support patients and families with their mental health and well-being.
They can support with:
- Adjustment or coping with illness, injury, and disability
- Death, grief, and loss
- Lifestyle changes
- Referral to community agencies
- Practical support and/ or financial concerns (e.g. equipment needs, information about income support programs)
