Geriatric Navigator (Dartmouth Community )

The Community Geriatric Navigator at Dartmouth General Hospital offers in-home health assessments to people living in Dartmouth who are frail and elderly (clients 65 years and older).

  • Assessments can include:
  • Cognitive testing
  • Mood assessment
  • Mobility assessment
  • Functional assessments (meal preparation, personal care, shopping needs, medication compliance)
  • Home safety
  • Assessment of community supports

How do I access this service?

If you would like to refer someone (or yourself) to the Geriatric Navigator, please call 902-465-8446, or complete this form and fax it to 902-465-3567. Clients do not require referral from a health professional for this service.

What will happen after my assessemnt?

Following the assessment, the geriatric navigator makes recommendations regarding community resources that may be helpful to allow people to remain in their homes and improve their quality of life.

A written report is sent to the family doctor.

Follow-up home visits are based on the client’s abilities and needs.
