Patient safety champion: Continuing Care Program helps clients navigate through global pandemic

This year, Nova Scotia Health recognizes Nova Scotia Health Continuing Care Nursing Program as one of the Patient Safety Champions for the additional service and support provided to clients during the COVID-19 pandemic. 

The Nova Scotia Health Continuing Care Nursing Program works internally with Nova Scotia Health to provide care to patients in communities across rural Nova Scotia.

A patient safety champion is an individual or team who has a genuine interest in safety and are respected by their peers by modeling Nova Scotia Health core values for patient safety. They are willing to embrace a leadership opportunity creating a ripple effect.

During the initial wave of the pandemic, health care services in many delivery areas were severely impacted - resulting in an increased demand in the community home care setting. 

The community home care program continued to provide service and support to clients when other areas were not available. They also advocated for clients and families who were vulnerable and unable to access other services.  

The team’s nurses became the 'navigators’ assisting clients and families impacted by the reduction of programs during the first wave of the pandemic. 

For example, navigating palliative care to provide increased supports to clients and their families who chose to die at home.

The team continued to provide front line care, taking on additional roles, such as in-home COVID-19 swabbing and filling gaps in other service delivery areas. 

The home care nurses always put the care, well-being and safety of clients and families first, being their access to health care at the ground level. 

Nominated by the team’s managers Jennifer Delorey, Angela M. Meagher and Wendy DeCoste, the team has more than 100 staff of RNs, LPNs and admin support, in 13 distinct offices, covering rural areas in both Central and Eastern Zones.
